In accordance with the Paperwork Burden Reduction Act, Clinton County will no longer be providing paper copies of the 1095-C forms for the previous tax year. If you would like a copy for your records, you may reach out to Pam Petersen at ppetersen@clintoncounty.ia.gov to receive a printed or electronic copy upon request.
As a reminder, the 1095 provides details about your health coverage including what months you had minimal essential coverage, and whether that coverage was considered affordable per the ACA. This form is not required for your IRS tax filing.
